Two companies have been fined after a worker at Drax Power Station in North Yorkshire suffered a broken foot when a 13kg weight dropped nearly four metres from an overhead crane.
Dangerous conditions
A Kent-based partner of a development company has been prosecuted after dangerous conditions were found at a demolition site in Dover. The Health and Safety Executive (HSE) prosecuted Allan Smith for failing to undertake an asbestos assessment and for not securing a demolition site.
False credentials
A Suffolk plumber who put lives at risk by illegally working on domestic gas appliances has been sentenced to six months in prison. Paul Jowett, 40, of Nelson Road, Pakefield, Lowestoft, admitted five breaches of gas and health and safety regulations when he appeared at Ipswich Crown Court.
Proper planning needed
A farmworker suffered severe facial injuries after falling 16ft through a shed roof in Cornwall. Bodmin Magistrates' Court heard self-employed worker, James Best, 24, of Blisland was asked to help remove fibre cement roof sheets from a shed at Park Farm, Washaway, near Bodmin on 12 July 2011.
Duty of care
Two Norfolk companies have been fined after a cleaner's hand was crushed in machinery. Richard Beane, 34, from Watton in Norfolk was working as a cleaner for SGA House Services Limited, which was contracted by Watton Produce Company Limited to work at their Attleborough vegetable packing plant.
Workplace aggression
A social care organisation has been fined for exposing workers to the risk of violence and aggression. The Health and Safety Executive (HSE) launched an investigation at Dimensions (UK) Ltd, a not-for-profit organisation that provides support services for people with learning disabilities, after a support worker was kicked in the eye by a client on 31 December 2009.
High risk activity
A Staffordshire-based aluminium casting company has been prosecuted after a man was burned by molten metal at its Worcester factory.
Common hazard
An international manufacturer has been prosecuted after a worker lost three fingers in a bubble-wrap making machine at a Buckinghamshire factory.
Lack of training
An Evesham horticultural nursery has been fined after a worker suffered serious head injuries and fractured his hand when he fell through a glasshouse roof.
String of failures
A Lichfield logistics firm has been fined after a forklift driver broke his neck when a computer cabinet he was loading onto a trailer fell on him.
